Note: The Qualification of the candidates must be from a recognised University/Institution. The application form may be downloaded from our website “www.doeaccchd.edu.in” or obtained personally on all working days and be submitted strictly in the prescribed proforma alongwith non refundable registration fee of Rs. 350/- in cash for each post applied separately before start of the interview. The candidates should attach copies of certificates/ mark sheets, Date of Birth certificate, passport size photograph, and experience certificates with the application form and should bring all testimonials/ certificates/ experience certificates in original at the time of interview.
